Sheffield health and Social Care NHS Foundation Trust (SHSC) has an exciting opportunity for a Patient Safety Investigator (with a professional registration) to work as a member of our ambitious team. We are looking for an exceptional individual who can demonstrate a real enthusiasm for patient safety. You will bring key organisational skills to incident investigation and patient safety projects within our ambitious programme of work.
Working in collaboration with the Clinical Directorates, the postholder will lead and/or facilitate corporate and clinical colleagues in the investigation of Serious Incidents, Structured Judgement Reviews and Complaints. The Serious Incident Framework is changing, and this role will adapt to facilitate Patient Safety Incident Investigations, as we implement the new Patient Safety Incident Response Framework.
The postholder will be expected to lead in the investigation of highly complex or contentious serious incident investigations (to become Patient Safety Incident Investigations) or complaints at the request of the Head of Clinical Governance and Risk.
The focus is to support trust-wide investigation, thematic overview and analysis and the learning lessons agenda by utilising appropriate investigation techniques, and critically analysing outcomes against required standards.
It is expected that the postholder will be/become a subject matter expert in the field of Patient Safety Incident Investigations (PSIIs) and have an excellent knowledge of the legislative and national frameworks and associated tools used for critical analysis.

- Attendance at SHSC’s Investigations Panel, with a responsibility to review developing investigation reports, offering advice and direction to investigations to ensure the Terms of Reference are met and that investigations are of a high standard and quality.
- Completing complex or contentious investigations at the request of the Head of Clinical Governance and Risk; in a timely manner in compliance with national requirements producing reports which are of a high quality.
- Develop Terms of Reference for serious incident investigations (PSIIs) and attend Investigations Panel to ensure the delivery of high quality investigations by colleagues from clinical and corporate services.
- Ensuring service users, their carers and families who have been affected by an incident are routinely invited to be part of the investigation process and share learnings from completed investigations as part of a candour process where appropriate
- Ensuring that SHSC’s Incident Management Policy and Procedures are adhered to in accordance with national best practice guided by the National Framework guides
- Ensuring Duty of Candour is applied and recorded appropriately
- Support the Patient Safety Specialist and Head of Clinical Governance and Risk to produce regular learning lessons reports and support learning forums and other mechanisms to ensure that learning from incidents is shared across SHSC and within the local system.
